Without seeing the jack on the back, this sounds like it is a Digital Equipment Corporation MMJ serial connector.
These used the standard 25 pin serial connector and broke out 6 leads to a modified 6 pin modular jack.
The release tab was offset to keep someone from plugging in an analog phone cable and accidentally damaging the serial port on the device.
This was used in structured wiring in the early days before everything used Ethernet.
